We invite you to the first session of our three-part Life & Legacy Planning series, focused on funeral planning with Rev. Monica and our Facilities Manager, Debby Tartell. Join us on Sunday, October 20, at 10:00 AM in the chapel to learn how to leave thoughtful service plans for your loved ones, secure an interment location in our Memorial Garden, and make other important decisions. Topics will include choosing hymns, readings, and key elements of the service. This series is designed to offer peace of mind and ensure your final wishes are honored. Part 2: Safety & Wellbeing Workshops for Caregivers, Elderly, & Disabled 

Sundays, Oct-27 & Nov-3, 10:00 AM

Hosted by Saint Matthew’s Health & Wellness Ministry, these workshops will focus on safety and wellbeing for caregivers, as well as for the elderly and disabled who live alone. We are pleased to welcome Denise Viker, BSN, RN, from Dignity Health Center for Faith Health Ministries.

Who will speak for you if you are unable?
Who will communicate your preferences to healthcare providers if you can’t? Are your preferences known?

Advance directives are essential documents that outline your healthcare and treatment wishes if you're unable to communicate them. Join us for a presentation that will guide you through the process of preparing and registering your advance directives with the Arizona Healthcare Directives Registry (AzHDR). You'll receive guidance on important topics such as:

  • Living Wills

  • Healthcare Power of Attorney

  • Mental Healthcare Power of Attorney

  • Pre-Hospital Medical Care Directives

  • Arizona Healthcare Directives Registry

Complimentary advance directive forms will be available to complete and register onsite, or you can bring previously completed forms to register with the AzHDR at no cost.
